I think it does have to do with not having running water. If you grow up in a home where you never flush the toilet, or can only flush it when someone pours water into the back of the toilet every once and awhile then its not part of your training, thus it doesn't connect. Also, yes, some cultural issues too. My son, a bio child, said when he first started wiping himself he forgot where the toilet paper went (I was thinking to myself, seriously, I didn't teach the child to throw it in the toilet?!). So, if no one ever tells them to put it in the toilet, or no one ever sends them back when they are 3 years old, or four years old, to flush and wipe correctly, they just don't know. Something like that probably does take a long time to relearn...signs might work, if they can't read, maybe a picture?
